Documentation for Service-Connected Mental Health Claims

What the Evaluation Includes

If you are pursuing disability benefits for a mental health condition, a psychological evaluation may be required as part of your claim. This service provides a comprehensive clinical assessment of your diagnosis, functional impairment, and the relationship between your condition and your military service.

Findings are documented in a written medical opinion prepared in accordance with VA evidentiary standards. Whether a claim is supported depends entirely on the clinical findings of the evaluation.

Each evaluation typically includes a review of relevant records, a comprehensive clinical interview, standardized psychological assessment when clinically indicated, and a written medical opinion addressing diagnosis, functional impairment, and service connection. A Disability Benefits Questionnaire (DBQ) may be completed when applicable.

Who May Benefit From This Evaluation

Consider scheduling an evaluation if:

  • You are filing an initial claim and your treating provider is unable to complete the required documentation

  • Your claim was denied and you are seeking additional medical evidence

  • The VA has requested clarification regarding your diagnosis or service connection

  • You want an objective, clinical assessment of your mental health condition in the context of your military service

Evaluations are available for a range of service-connected mental health conditions, including PTSD, depression, anxiety, and adjustment disorders, among others. This service is self-pay and not billed to insurance.
